Yes, it is possible, with (1) smart, motivated developers whose task it 
is to build a highly-scalable solution, (2) good admins who know how to 
operationalize a service like this, and (3) enough hardware.

Neither of those is trivial, but they can be overcome.

Note that the open-source developers who have worked on Jabber/XMPP 
server so far are pretty smart, but their task has not been to develop 
highly-scalable servers (that's a big-company itch!). Plus they don't 
have access to lots of hardware, and usually don't have experience with 
operationalizing services for millions of people (IMHO that's a rather 
specialized area of knowledge, mostly acquired by people at large ISPs, 
telecoms, and portals -- more big companies!). But there is nothing 
inherent in Jabber/XMPP technologies that makes it impossible to build 
an IM service for a million concurrent users.
