In general, there will be two entities involved in any data forms 
transaction:

"Thing One" defines the form, processes submitted forms, and returns 
results. 

"Thing Two" receives empty forms, submits forms, and receives results.

Perhaps the terms I used are a bit confusing, because "Thing One" can
"provide" data via form results.

Would the terms "form processing entity" and "form submitting entity" be
clearer?
