If you're not already considering it, you should take a look at the
free speex voice codec. Fewer strings attached.

Dealing with NAT and firewall traversal without requiring user
configuration or a central server seems to be where most of the
action is, although you already have a central server (or at
least an addressable network of them) for XMPP.
For audio and video chat, people prefer that because it reduces
latency and the load on the central server.
