This isn't the only way to send it.  Spammers often have their own mail 
servers.  Clients can always connect directly to the receiving server too.

Jabber solves the issue of clients spamming directly to servers, but not the 
problem of spammers having their own valid domains.  However, things such as 
dialback do in fact slow down the spammers and make their activities cost 
more, which is good.

It's an expected use case where the message should be delivered as per usual.  
Some clients offer the ability to drop messages from users not on the contact 
list, but in general it's good to be able to receive messages from new users.

Spam might, of course, come from people who managed to get on the contact 
list.  Also, as someone else pointed out, the request to get on the contact 
list can itself be a spam.
