That's true. H.323 is not very NAT friendly. But it won't work to send
the VoIP data through the server. At least if you use one of the public
servers. They all have something that is called "karma". If a users
starts sending data to fast they will limit the bandwidth of this user.

If you don't want to use a gatekeeper for your NATed network, you can
also enable IPv6 on your network. With every public IP address you get
2 to the power of 80 public IPv6 addresses. This is enough for your home
network and you don't need NAT ;-)
You can then use a H.323 client that supports IPv6: GnomeICU.
