We did it for one of our clients.
For us it was purely server side solution. When somebody disconnects
he can set on server that all the messages that comes to him
will be redirected to sms gateway - and then sent to the
gsm phone. Like this the person could be online all the time.

Of course we could also do the direct sending of sms'es
the problem was that our customer wanted to limit as much
as possible the number of smses sent trough his gateway.
