Having been "involved" (and I use that term loosely) with
the openH323 effort for a couple of years now,  I will
second the statement that openH323 is rather well
implemented.  But it is not for the faint of heart.

The openH323 codebase has some very simple programs that can
be used to create H323 VoIP calls inside a custom jabber
client.  It should be pretty easy to use their codebase to
create an h323 call between 2 users.  However, you will need
the IP address for whom you wish to call.  Your  client must
also have a "listener" monitor the H323 port so that it can
accept the call coming in.

The openH323 code is portable to windows and several flavors
of *nix, so you have your choice of platforms to develop
your client on.

There is even a PlamOS port in the works -- might be nice to
have a client on the Palm that does instant messaging and
VoIP all in one client :-)

They have the h.323 stack rather well implemented, although
that
technology is starting to slide out of the light.  You can
get a lot
done with basic GSM and RTP.
